ATTENTION 5TH GRADE PARENTS AND GUARDIANS!!

It is hard to believe this will be our last year at Creek View Elementary. A committee of volunteer
parents has started planning all the FUN for our 5th graders as they wrap up their time at CVES
and move on up to middle school!!

The 5th Grade Festivities Committee is working with the PTA to collect a $30 donation by Friday,
November 7, 2025 from each 5th grade student. Our goal this year is to have 100% participation
from our 5th grade class. Last year we had 100% participation! The donations will be used to fund
the many activities planned to make your student’s final year of elementary school memorable.

Your donation will make the following possible:

Fun Fridays: During school hours starting in April
Farewell to CVES Week: Spirit Days during last week of school
Outdoor team game with Teachers and ice cream
Promotion Ceremony: 5/20 (In the morning at school)
Glow Party: 5/21 (during school hours)
5th grade T-shirt (To be worn on Field Trips, Fun Fridays, Spirit
Days)

All $30 donations submitted by Friday, November 7, 2025 at PTA’S WEBSITE will guarantee your child a 5th grade T-shirt. You must have a PTA account and log into your account to see the payment link at “My
Forms/Paperwork” under “My Account"! If you need to create a PTA account, follow the step-by-
step registration process in the "CVES PTA Account Creation" process shown below this letter.

Additionally, we are asking each family to send 2 photos of their student:
1. A baby photo (or oldest photo you have)
2. A current photo depicting something that makes your student unique. Examples: sports, hobbies, reading, music, art, favorite team colors, cultural celebrations, etc. This photo must be of your student ONLY - no family members or friends.

Please submit your photos to cves5thgraderpics@gmail.com and include your student’s name
and homeroom teacher in the subject line.
